The piriformis muscle is a topic of special interest in manual
medicine. That muscle can be affected by tension, spasm or trigger
points or taut bands, aech of them a potential source of pain. The
pain can be local, in the buttock, or referred to the tigh, maybe
because of the close relationship between the muscle and the trunk
of the sciatic nerve.
The piriformis muscle can be readiliy palpated right in the middle
of the buttock, in the center of the triangle made by the PSIS, the
tip of the sacrum and the greater trochanter. This central point
does correspond to the emergence of the sciatic nerve, and one can
wonder whether it is the muscle or the nerve which hurts at
palpation.
