Saignée et sciatique
Bloodletting and sciatica

 

Voici une description de la sciatique dans un manuscript du XV° siècle.
"La goutte en la hanche, dicte sciatique. Il est une goutte que l'on appelle sciatique, laquelle tient en la hanche et est engendrée des humeurs qui descendent au gros nerf qui est entre les jointures de la hanche ou de la cuisse (...) Cette goutte est souvent causée des humeurs gluens et tenans qui s'assemblent au creux de la hanche (...) et ces choses sont cause des douleurs qui descendent souvent es cuisses et es membres jusqu'aux talons et jusques au petit doigt du pied..."

L'irradiation à l'orteil est caractéristique de la sciatique. Il était courant, à cette époque, de faire de la sciatique une maladie de la hanche et de la rattacher à la goutte. Du reste, le nom même de la sciatique vient du latin ischia, la hanche. Ainsi Shakespeare : "How now, which of your hips has the most profound sciatica?" dans Measure for Measure, act 1, sc. 2).

Here is the description of sciatica in a manuscript from the XVth century.
"The gout in the hip, so called sciatica. There is a gout, called sciatica, taking the hip, which is caused by humors going down to the big nerve between the hip joint and the thight (...) This gout is caused by sticky humors assembling into the hip (...) and these things are cause of aches going down to the thight and leg, to the heels and to the little toe of the foot..."

The radiation to the toe is typical of sciatica. It was common, in the old ages, to consider sciatica as a hip joint disease, due to the gout. Besides, the name of sciatica comes from the Latin word ischia, meaning hip. And Shakespeare: "How now, which of your hips has the most profound sciatica?" in Measure for Measure, act 1, sc. 2)

L'image nous montre que le traitement proposé dans ce manuscript est la saignée.

Voici un extrait d'un ouvrage médical, intitulé "Avis au peuple sur sa santé" écrit par le médecin suisse Simon-André TISSOT (1728-1797). [L'une des indications des saignées est] "quelquefois pour appaiser une douleur excessive, qui ne dépend point cependant de trop de sang, ou d'un sang enflammé, mais qu'on calme un peu par la saignée, afin d'avoir le tems de détruire la cause par d'autres remèdes".

The illustration shows us that the treatement suggested in this manuscript is bloodletting.

Here is a quotation from a medical textbook by S.A. Tissot (1728-1797). [One of the indications for bloodletting is] "sometimes to relief excessive pain, even if it does not depend upon too much blood or inflammed blood. It gives time to address the real cause of the pain and to treat it by other means".

La saignée se pratiquait à l'aide d'une lancette. Pour la sciatique, elle était pratiquée au pied.

Bloodletting was performed by the mean of a lancet. For sciatica, it was done on the foot.
La saignée fut remplacée au XIX° siècle par l'application de sangsues sur la jambe, qui se nourissaient de sang en l'aspirant.

Bloodletting was replaced by the application of leechs on the leg during the XIXth century. This treatment is called leeching.
